GolfSchool District 67

A student's parent(s)/guardian(s) shall present proof that the student received a health examination with proof of the immunizations against, and screenings for, preventable communicable diseases, as required by the Department of Public Health. Unless an exemption or extension applies, the required health forms must be presented to the district by October 15 of the current school year in order for a student to attend school.

• Students entering an Illinois school for the first time:
• Illinois Child Health Examination Form
• Vision Examination Form

 

• Students entering kindergarten:

• Vision Examination Form
• Dental Examination Form

 

• Students entering 1st grade (If they did not attend kindergarten):

• Vision Examination Form

 

• Students entering 2nd grade:
• Dental Examination Form

 

• Students entering 6th grade:

• Dental Examination Form
